What is it?

Form Builder is an open-source tool that helps you create print-ready forms for in-class evaluations, feedback, quizzes, and tests. With its WYSIWYG editor, you can create and edit forms on the fly as well as drag-and-drop questions from a list of previous questions written by you or shared by the UBC community.

Just login with your CWL at http://formbuilder.ctlt.ubc.ca to start building!

Go to http://formbuilder.ctlt.ubc.ca and login with your CWL.

Tips

Below are some general tips about Form Builder.

  • Save your forms often as you get timed out after a time without any activity.
  • You can only edit your questions in the Question Bank.
  • If you want to slightly modify a shared question, you will need to add it to your form first.
  • Write specific titles for your forms to help you remember when you used them.
