Form Builder is an online form-building application that helps instructors create print-ready forms for in-class evaluations, feedback, quizzes, and tests. Instructors can create and edit forms from scratch or build from previous questions and common questions shared by the UBC community.
How do I get started?
Log in to the formbuilder.ctlt.ubc.ca to start creating or editing forms and questions.
Form Builder has a basic tour to orient you inside the application. Click the '?' icon once you have logged in to access this feature. Alternatively, you can find some basic Form Builder how-tos in the Wiki.
What support is available?
Form Builder is a tool developed within UBC's Centre for Teaching, Learning and Technology. Contact firstname.lastname@example.org with questions. Support is provided by the Learning Technology Hub and individual instructional support units.
